What to Bring
An assessment or intake, by UCS Healthcare or another agency, is required before a patient begins to receive services.
Assessments at our West Des Moines location can be completed with no appointment on a walk-in basis Monday - Friday between 8:00 a.m. and 2:00 p.m. Please check the location page for each location for walk-in assessment hours.
Individual and group treatment services are available six days a week based on individualized needs.
Group schedules are published weekly and available from our Patient Support Services desk.
When you come to UCS Healthcare for our services, there are just a few items you'll want to bring with you.
- Photo identification (Driver's License, government-issued ID, etc.)
- Proof of income. This can be any of the following - a pay stub from the last 30 days, last year's tax return, Wage Verification Form "White Sheet" from Iowa Workforce Development
- Insurance or Medicaid Card (if you have insurance)
If you are taking prescription drugs or vitamins/supplements, it's always a good idea to bring them to your first appointment so we can obtain a full medical history.
