Substance Use Disorder Intake/Assessment Hours are Monday through Friday, 8:00 a.m. to 2:00 p.m., in our West Des Moines Location. 

 Substance Use Disorder Intake/Assessment Hours are Monday through Friday, 11:30 a.m. to 1:30 p.m., in our Ankeny Location.

Substance Use Disorder Intake/Assessment Hours are Monday through Friday, 11:30 a.m. to 1:30 p.m., in our Knoxville location. 

 

Our substance use disorder assessments are a walk-in service. There may be a wait, but we will ask you to fill out a questionnaire. The intake process may take 20 minutes. Please bring your insurance card and your last pay stub.
